Anime ini bercerita tentang dua remaja bernama Mitsuha Miyamizu dan Taki Tachibana yang bertukar tubuh karena sudah kejadian. Awalnya mereka tidak saling mengenal dan keduanya tinggal di tempat yang berbeda jauh.
Mitsuha Miyamizu tinggal di daerah pegunungan di pedalaman Gizu, desa Itomori. Dia sendiri merasa bosan dengan kehidupannya di desa. Sedangkan Taki Tachibana tinggal di pusat kota Tokyo, tepatnya Shinjuku yang bekerja secara freelance di sebuah restoran Italia.
Pada suatu hari keduanya dikejutkan dengan sebuah kejadian yang nggak masuk akal. Tubuh mereka tertukar hingga membuat keduanya sangat terkejut. Karena saling tidak kenal, mereka harus mencari petunjuk sendiri untuk bisa menemukan tubuh asli mereka.
Mereka bahkan menulis nama di tangan masing-masing agar tidak lupa. Suatu hari mereka berpapasan di tengah jalan tanpa saling mengenal. Delapan tahun kemudian, mereka kembali bertemu dan saling menanyakan nama. Sisanya harus kamu tonton sendiri. Kisah menyentuh ini akan membuat hati kalian campur aduk.
